"I give the Peruvian American Medical Society Endowment Fund, Tax ID# 38-2824781, (all, or ___ percent of the rest, residue and remainder) of my estate for its general purposes."

Naming PAMS as a beneficiary is a simple and powerful way to support the life-changing work of our organization without updating your will.  All beneficiary designation gifts pass directly to PAMS without going through the probate process. 

Contact your plan administrator or financial institution for a change of beneficiary form.  Or visit the beneficiary page on their website where you can complete the form in just a few minutes.  On the beneficiary form, decide what percentage, up to 100%, you would like us to receive.

Gift of Life Insurance

You can name PAMS as a beneficiary of your life insurance policy for any percentage up to 100% by submitting an updated beneficiary designation form with the policy holder. You can also make PAMS a contingent beneficiary, which means that we will receive the balance.
